Due to the outbreak of the Coronavirus (COVID-19), RMNOW is taking extra precautions with the care of every client to include enhanced sanitation/disinfection procedures.
Symptoms of COVID-19 include but are not limited to:
High temperature
Sore throat
Headaches
Loss of smell / taste
Fatigue
Dry Cough
Difficulty Breathing

I understand the above symptoms and affirm that I, as well as all household members, do not currently have, nor have experienced the symptoms listed above WITHIN THE LAST 14 DAYS.
I affirm that I, as well as all household members, have not been diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.
I affirm that I, as well as all household members, have not knowingly been exposed to anyone diagnosed with COVID-19 WITHIN THE PAST 30 DAYS.
I affirm that I, as well as all household members, have not traveled outside of the country, or to any city considered to be a "hot spot" for COVID-19 infections WITHIN THE PAST 30 DAYS.
I understand that RMNOW cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form.
I understand that RMNOW cannot guarantee that myself or / and my child(ren) will not become infected with COVID-19.
RMNOW is following these enhanced procedures to prevent the spread of COVID-19:
Product delivery can be contactless and picked up from the studio. Please allow additional time to fulfil orders during the pandemic.
Additional time included between appointments to prevent client contact with each other.
Guests accompanying clients is not permitted unless previously arranged with the studio.
Where possible we are limiting photo sessions to one household families.
Each client required to wash hands/ Sanitise upon arrival and before leaving.
No footwear to be worn inside the studio by either the photographer nor the client. You will be asked to remove your shoes at the door. If you wish to wear footwear to your session they must be cleaned and brought in in a separate bag.
Your photographer will wear a mask at request and thoroughly clean their hands and use sanitiser during the whole session. Clients must bring in a mask to be worn throughout the session (with an exception of when the photos of yourself are being taken).
All photography items and props used during your session will be thoroughly disinfected and disposable items will be used when possible.
All surfaces will be wiped thoroughly with disinfectant before and after each client according to the manufacturer's directions.
By signing below, I acknowledge the contagious nature of COVID-19 and I agree to each statement above and release RMNOW from any and all liability for the unintentional exposure or harm due to COVID-19.
I voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending the photoshoot and that such exposure or infection may result in personal injury, illness, permanent disability and death.
I understand that the risk of becoming exposed to or infected by COVID-19 at RMNOW studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to the photographer and other studio employees and contractors, such as assistants and make up artists if those are present.
I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the photoshoot with RMNOW.
On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless RMNOW, its employees from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto.
I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the RMNOW, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any business related activities during our session.
RMNOW agrees to abide by these standards and affirms the same.
